Ask an Expert: Medications and Your Health

Latest Question

When should I take Synthroid®, first thing in the morning or just before I go to bed?

Daniel Kim's Response

It's generally recommended that levothyroxine (Synthroid®) be taken daily on an empty stomach, at least 30 minutes before food. The reason for this is that if you eat certain foods they may interact with levothyroxine, and less medication will make its way into the body. Your doctor may need to change your dose if absorption is impacted.

Some examples of foods that are known to bind to and prevent absorption of levothyroxine are:

  • calcium and calcium-fortified orange juice
  • cotton seed meal
  • dietary fibre
  • soybean flour (infant formula)
  • walnuts

You should take this medication on an empty stomach to minimize any impact on absorption.

While most recommend taking the medication in the morning, you can also take it on an empty stomach at bedtime. A recent study confirmed that taking levothyroxine daily just before going to bed was an acceptable and effective alternative.

It is important that you are consistent in how you take your medication. You should take your medication in the same way and at approximately the same time each day.
